Transition to adulthood
Moving from children's health services to adult services is called transitioning.
Transition gives you, and everyone involved in your care, time to make sure you're ready for adult services and to discuss what your healthcare needs might be.
Leaving a team of doctors and nurses you've been with for many years can be overwhelming, but hopefully, by getting involved in the transition process, you'll feel more confident and happier about it.
