Request an education, health and care needs assessment
To request an education, health, and care needs assessment (EHCNA), you must create an account. If you already have an account, you can skip this step.
By creating an account, you can save your request and come back to it later if you need to. To set up your account, you must:
- let us know your email
- create a password
- confirm your account by clicking a link sent to your inbox.
