As a result of a KMCR, they will be able to see:
- the care you currently have
- any medical conditions
- any medications.
You won't have to share your medical or care history every time you see a health or social care professional. Also we can:
- improve the safety of your information
- offer accurate information
- supply the information to each professional quickly
- improve the quality of your care.
