Are you looking for a great opportunity to join a new social care community team in Kent?
We are setting up new social care team, which allows workers to spend more time with clients, supporting and empowering them to become as independent as possible.
As part of the Transforming Integrated Care in the Community (TICC) project, we aim to bring the principles of the Buurtzorg model from the Netherlands to the UK. We are building our own Buurtzorg style community team in Ashford.
The Buurtzorg model has revolutionised community care in the Netherlands and their nurse led teams:
- are self managed
- have the freedom to make their own decision as a team
- have an equal voice supported by their coach
- are small and have a caseload of approximately 40 clients concentrated in a limited geographical area and spend 60% of their time with their clients.
We are looking for Care Workers and Enablement Support Workers to join this team alongside Occupational Therapists and will be supported by a coach. The team will cover the hours of 7am – 10pm across a 7-day week.
The post is funded by Kent County Council and we are offering both full and part time roles on an 18-month contract with a possible extension of a further 6 months.
