Apply for an older or disabled person’s bus pass
You can apply for a bus pass:
You will need to provide your details, proof of eligibility and
a suitable photograph.
Online
The quickest way to apply for an older person’s bus pass is
online. You can take a photo with a webcam, if you have one, or
upload a digital photo you have stored on your computer, and you
won’t have to provide copies of any supporting
documents.
In person
There are a number of places where you can go to make your
application. Remember to take proof of eligibility and a photograph
with you:

other libraries and Gateways:

- Call us on 08458 247 802 between 9.00am and 5.00pm, Monday to
Friday
- Give us your details over the telephone and you will be asked
to post us your proofs of eligibility and photograph
The required proofs are different depending on what type of bus
pass you are applying for.
Post
You will need to print the form you need:
Once you have filled in the form you can post it to us, along
with proofs of eligibility and a suitable photograph, or you can
take it to any library or Gateway. Send it to:

1XX.
Required evidence
To process all new applications you need to provide good quality
photocopies of eligibility evidence and a passport photograph,
taken within the last 6 months.
Only send photocopies of documents to us – please do not send
originals. We cannot accept responsibility for any original
documents sent or guarantee that they will be returned to
you. You need to put your name and postcode on all photocopies
of documents.
