Common Assessment Framework
What is a common assessment and how can it help your
family?
The Common Assessment
can, with your agreement, help you to support your child and
family. Starting with a conversation about what is working well in
your family and where some support might be needed we can work
together with you to look at how things could be improved.
How assessment works
- You or someone working with your child may be concerned that
he/she is having problems. We will talk to you and may suggest
that a common assessment may help.
- Someone who works with your child will meet with you to discuss
what's going well, what needs to improve and what you and your
child want to achieve. They will then complete the standard
form with you and you agree who this information is shared
with.
- You and services working with you will agree an action plan. If
more than one service is helping we will form a team around your
child. You and your child are important members of this team. You
will jointly agree which member of this team will be the Lead
Professional.
- You will meet with the team to review progress. this may lead
to new actions, closure if outcomes are achieved or suggestions
that your child needs more specialised help.
- Your assessment will be closed once your outcomes have
been achieved.
