Rehabilitation and recuperative care
Rehabilitation provides people with physical and sensory
disabilities with help to recover and regain their independence
often following an illness or injury/stay in hospital.
A case worker will provide advice on how to cope with illness or
disability, or organise the provision of aids and equipment to help
in everyday tasks.
- Meals delivered to your own home
Sometimes people are not able to cook for themselves, because of
illness or frailty, and this can put at risk their ability to
continue living independently in their own homes. To help them
overcome this problem, our Social Care teams can arrange for
community meals to be delivered direct to the doors of those
assessed to be in need of the service.
- Equipment and adaptive aids
The role of the Occupational Therapy Service is to assess the needs
of people of all ages who have difficulty managing activities of
daily living due to a physical disability or frailty for special
equipment and/or adaptations to their home, which will increase
their control over the practical aspects of their everyday
life.
- Older person's rehabilitation
Recuperative Care is aimed at older people who need a period of
rehabilitation within a safe and supportive environment.

The online Self-assessment application provided by Social Services
gives people the choice to assess their own care needs or carry out
an assessment on behalf of someone else. If it is agreed that you
qualify for support, you will be able to choose from a range of
support services matching your assessed needs
- Make a referral to social services
If you would like help or assistance from Social Services for
yourself, a friend or relative, you can contact us to make a
referral. If you are enquiring on behalf of someone else you must
have their permission to contact us.
