Independence Through the Voluntary Action of Kent's Elders
(INVOKE) is an east
Families and Social Care project that supports the residents of
Ashford, Shepway, Canterbury, Swale, Thanet and Dover to live
independently in their communities.
It is a multi-partnership project that includes voluntary
sector,
Eastern and Coastal Kent Primary Care Trust and Familes and
Social Care, with the support of east Kent residents .
The project consists of two services.
1. Community Matron Support Workers
(CMSW)
Community Matron Support Workers (CMSW) are accessible via the
Community Matron Service working with persons who have long
term conditions. Please contact your local GP if you wish to find
out about this service.
2. Care Navigator Service
Care Navigator Service will visit you in your own home,
exploring with you the range of solutions that will help you to
resolve your problem/need. Please contact you local Care Navigator
