You can use the internet to find out if Kent County Council
can provide you with help or support in your daily activities or
provide support for you as a carer. This is called self-assessment.
Self-assessment gives people the choice to assess their own needs
or carry out an assessment on behalf of someone else.
Who can use this service and further information?
You can find out more about assessing
your care and support needs, who is eligible for support and what services may be
offered. We recommend that you visit this section before you start
your self-assessment.
A self-assessment can be completed by anyone aged 18 or over,
who lives within Kent County Council's boundary and who is having
difficulty carrying out everyday activities.
Carers, friends or family can complete the self-assessment on
behalf of someone else. This person must be aged 18 or over and
live within Kent County Council's boundary. In this case, we will
always contact the person concerned to check that they are in
agreement.

Giving you the choice and providing a faster response
After you complete a self-assessment you will receive an instant
response informing if you, or the person you are completing the
assessment for, are eligible for services. We might recommend a
further assessment for people who have more complex needs than we
cover in the self-assessment. This will involve meeting the person
who is being assessed to give them the opportunity to discuss their
needs and wishes. We always encourage relatives or carers to be
involved in this discussion. A further assessment does not involve
a medical examination and we only speak to a person's doctor if
they have given us permission.
If we agree that you qualify for support, you will be able to
choose from a range of equipment or services matching your assessed
need. If you would prefer, we can arrange for you to have the cash
equivalent for some of these services so that you can arrange them
yourself. This is called a Direct
Payment.
Types of Self-assessment available
There are two types of self-assessment available. A general
needs assessment and an assessment for carers. Please read the
descriptions below before you decide which assessment is
suitable.
General Needs Assessment
This assessment is for adults who are finding it difficult to
manage everyday activities. For example you or someone you know may
be having difficulty managing steps or stairs, getting in and out
of bed or some aspects of personal care. You will be asked
questions to help us understand your needs and what, if any,
equipment or services we can provide to help you.

Carer's Assessment
Carers look after family, partners and friends in need of help
because they are ill, frail or have a disability. This assessment
is for Carers and enables a person to assess their needs as a
carer. You will be asked questions about the person you care for,
and the type of care you provide. This will help us understand your
needs and whether we can provide you with help and support in your
capacity as a carer.
