Better Homes Active Lives - Business Information Questionnaire

Please note this tender has now closed - this page is for information only.

Kent Adult Social Services requires providers of domiciliary care to complete the following Business Information Questionnaire in order to apply for or renew their existing Approved Provider Status. Only providers with new or renewed Approved Provider Status will qualify for tendering for domiciliary care services for the 7 new Extra Care Housing developments for older people which are a part of the Better Homes Active Lives PFI.

Those organisations who do not wish to tender, will be offered an opportunity to apply for the new or renewed Approved Provider Status at a later date.

Please note that the Business Information Questionnaire must be completed and submitted electronically to proceed to the next stage of the domiciliary care tender. The questions you will be asked in the Business Information Questionnaire include:

  • the district zones you are interested in tendering for
  • the type of service you represent
  • the type of organisation you are
  • legal status
  • equalities legislation
  • undertaking of the parties

The Business Information Questionnaire for Better Homes Active Lives PFI is designed to be easy to complete. Once completed and submitted you will receive a copy of your completed questionnaire by email.

In addition to the Business Information Questionnaire, organisations interested in tendering must also provide:

  1. a copy of your most recent 'service user survey' report
  2. Annual Quality Assurance Assessment
  3. audited accounts for the last two year
  4. details of the contract department of a maximum of three local authorities (other than Kent) for which your organisation has provided services. These details must be provided to us by organisations who do not currently hold a KCC contract, by Tuesday, 14th October 2008.

Organisations' eligibility for Approved Provider Status will be measured against the above and CSCI reports, which will be accessed by KCC as part of the evaluation process.
