Who is self-assessment for?

The occupational therapy self-assessment can be done by anyone living within Kent County Council's boundary, aged 18 or over who has difficulty carrying out everyday activities.

Carers, friends or family can also complete the assessment on behalf of someone else living in Kent. If this is the case, we will always contact the person concerned to check that they wish to have the services we may offer.

What sort of services might I be offered?

The kinds of equipment or home adaptations we are able to arrange for people after a self-assessment include:

  • Toilet frames and raised toilet seats, to help you get on and off the toilet.
  • Bed rails and bed raisers, to help you get in and out of bed more easily.
  • Chair raisers, to help you get in and out of your chair more easily.
  • Grab rails, for example, by your front door or by a toilet.
  • Extra stair rails so that you are able to climb the stairs more easily or safely.
  • Kitchen stools and kitchen trolleys, to help you sit in the kitchen to prepare food and then to be able to carry it safely to where you wish to eat.

Is there any cost to me?

There is no charge for any equipment services or any of the home adaptation services that we offer through self-assessment.

What happens next?

When you have completed the assessment, you may be given options for services, depending on how you have answered the questions. If you would like any of those services to be arranged for you, you will need to tick the relevant box on the screen.

You will also be given a unique reference number, which you should make a note of in case you have any further queries.

Your assessment will be sent to your local area Occupational Therapy Service team, where it will be reviewed by a qualified occupational therapist. In most cases, the occupational therapist will arrange for the equipment or adaptations to be provided. We will aim to have these in place for you within seven working days after we have reviewed your assessment.

We will always make an appointment with you to arrange delivery and fitting of equipment or for fitting of adaptations.

If there is anything that we need to check with you before we go ahead and arrange services or if we need to arrange for a further face to face assessment at your home, we will get in touch with you.
