Can I complete an assessment on behalf of someone else?

If you believe that a relative or friend or someone that you are caring for (living in Kent), may have a need for occupational therapy equipment or services, then you can ask us for an assessment or complete an assessment on their behalf. In the first instance we will try and contact the person who requested the assessment. If you do complete an assessment on behalf of someone else living in Kent, we will always contact the person concerned to check that they wish to receive the services we may offer.
