Who comes to a Family Group Conference?

Your family network

Your family network may consist of children, grandchildren, relatives, friends and neighbours, or anyone you would like to attend and who will support you in making decisions towards your care. As the person the FGC is for, you decide who you want to include.


Professionals

People who work with you such as a Care Manager, District Nurse, Occupational Therapist, Doctor or any Community Health or housing representative. These Professionals leave the meeting before Private Family Time.


An Independent Co-ordinator

This is someone who will help you through the meeting. The Co-ordinator will organise and explain everything and help sort out any difficulties. The Co-ordinator also leaves the meeting during the private family time, but can be called in if needed. Remember - it is your meeting. Be sure to tell the Co-ordinator the names of everyone you think should be involved.


An Advocate

An advocate is someone who can assist you to get your views heard. They can help you say what you want to say or speak on your behalf if this is helpful. An advocate can be someone you know or a person specially trained as an advocate. Your FGC Co-ordinator can help arrange an advocate if you would like to have one.
