Referrals can be made by statutory agencies such as
housing departments and social services, voluntary agencies,
support providers and vulnerable people themselves.
To make a referral for floating support you need to complete a
floating support referral form. Guidance on how to complete the
form is also given.
You can refer yourself by
filling in this form and emailing it to
Supporting People.
You can refer yourself to accommodation based services by
contacting the provider of the service directly. Contact details
are available via the
support
services directory.
Who decides what support is needed
When the Floating Support team agrees that you qualify for help,
you and your support worker will together agree a support plan that
details what you want to achieve. The support plan will identify
who is responsible for certain actions, how the support will be
provided and for how long. The plan will be regularly reviewed and
you can involve other people in deciding what you want.
When you and your support worker agree that you no longer need
support, the support will then go on to someone else who needs it.
However, should you feel you need support again in the future, just
get in touch and we will take a new referral.
In Kent there are a number of providers who will decide whether
or not they can provide you with the appropriate support.
Alternatively, we can decide which agency is most suitable for your
needs.
