Important - please read
The services available from self-assessment are available to
people 18 years or over, who live within Kent County Council's
area. If a person wanting to be assessed lives in Medway you will
need to contact
Medway Social Services directly, not Kent
County Council.
Kent Adult Social Services cannot guarantee that a particular
service selected will be available. If the service you have
selected is not available, alternative support will be
provided.
We strongly recommend that you read the
frequently asked questions before starting your
self-assessment.
On receipt of a completed assessment social services will make
an electronic record of the information received. This will mean
that we will retain copies within the organisation of the
information given. Each person the carer supports will have an
electronic record created in their name. Social Services have a
policy which covers
Data Protection and confidentiality.
Declaration
I understand that in order for Kent Adult Social Services to
arrange services, the information I provide in the self-assessment
may be shared with other professionals who may be involved in
helping meet my needs or the needs of someone I care for.
If I am completing an assessment on behalf of someone else by
continuing with the assessment I confirm that they are aware that I
am doing this on their behalf and I have informed them that an
electronic record will be created in their name and information
about them may be shared with other professionals in order to help
meet their needs.
